The website will initially bring you to the Japanese site, select "multilingual" at the top center of the page for translation to English.

Multilingual Medical Questionnaire: Should you need assistance when reaching out to local facilities the following link can assist you with translation https://www.kifip.org/medical/english/index.html. This can be a useful tool for conveying how you feel to a local facility that may not speak English.  Once on the website there is a list of medical questionnaires that you can bring with you as an additional translation tool.
